Responsibilities:
- Provide strengths‑based, outcome‑focused social work interventions, supporting recovery, resilience, social inclusion, and community engagement for adults with complex mental health problems.
- Conduct needs assessment, develop and review care and support plans for adults with complex mental health issues and potential co‑occurring problems not receiving secondary services.
- Collaborate with the Adult Social Care Mental Health Team and ARC to ensure effective service operation and contribute to its ongoing development.
- Protect children, young people or adults at risk, in line with council responsibilities.
Qualifications and Requirements:
- Qualified Social Worker with experience working with adults experiencing mental health difficulties.
- Valid driving licence.
- Enhanced Children and Adults Barred list criminal records check (DBS) is required.
- Ability to greet customers in Welsh.
