The people we support can have long term complex challenges around their physical health, mental health and emotional well-being. It works with both older and younger adults. We aim to provide holistic support to allow individuals to live their lives as they wish to and ensure voice, choice, independence and control in their lives.
We are seeking to appoint a creative, enthusiastic, ambitious full time permanent OT within the Integrated Cluster Networks. Using a strength-based outcome focused model, the teams support adults and their carers to achieve positive outcomes within their lives and encourage wider inclusion in local communities. The particular focus of this role is to maximise the independence of individuals who are in receipt of managed care support and to ensure that the level of commissioned care is proportionate to individuals' capabilities. We specifically wish to target those individuals who have long-term reablement goals/outcomes. We are keen to continue to develop innovative approaches to OT practice.
We work closely with GPs and other cluster partners and utilise the Contingency Care Model to prevent unnecessary admissions to hospital and care homes, as part of our role to maximise independence and choice.
