This is an exciting opportunity for an individual to join the Home Treatment Team as a Community Support Worker, based in the Princess of Wales Hospital. This is a specialist service for adults with a severe and enduring mental illness and possible co‑occurring issues including substance misuse issues. The team covers the Borough of Bridgend.
The aim of the Home Treatment team is to support people to live in their chosen environment by preventing avoidable hospital admission and to support discharge from hospital. Adhering to a strengths‑based model of practice and outcome‑focused assessments, your role will be to work closely with individuals, supporting them in their recovery following a presentation in crisis, in the community and/or in hospital, and support these individuals and their carers to improve and maintain independence at home or assist in an early discharge from hospital.
This will include actively completing brief interventions with individuals to keep them independent, signposting to services and working closely with professionals and the team. Working alongside a team of multi‑disciplinary professionals, you will provide dedicated recovery‑focused intervention and support to individuals having mental health or accompanying substance misuse issues as a result of their prevailing social situation.
You will be line managed by the Senior Social Work Practitioner based in the Home Treatment Team.
The ability to greet customers through the medium of Welsh is a requirement for this post. Protecting children, young people or adults at risk is a core responsibility of all council employees. An Enhanced Children and Adults Barred list criminal records check by the Disclosure & Barring Service (DBS) is a requirement for this post. A valid driving licence is a requirement for this post.
