Our 15+ Team currently has an opportunity for a personal adviser to join them. The 15+ Team work to achieve the best long-term outcomes for the young people they work with. The team strives to support young people making their transition into adulthood an easier and manageable experience.

Responsibilities

  • Work in partnership with young people, their carers and other professionals to complete high-quality assessments in order to develop outcome-focused Pathway Plans.
  • Work alongside social workers supporting care-experienced young people undertaking needs assessments with a focus on transition.
  • Commit to supporting positive outcomes and a person-centred approach.
  • Protect children, young people or adults at risk, a core responsibility of all council employees.
  • Follow the Signs of Safety model to guide practice and the services’ outcomes-focused approach; engage in training to use this methodology.
  • Adhere to the requirements of the Social Services & Well-being (Wales) Act 2014 and safeguarding of the most vulnerable children and adults.
  • Apply the council’s Hybrid Working Policy, establishing how you will undertake working hours between your home and the office.

Qualifications and Requirements

  • Recognised qualification in social care and experience of working with young people aged between 15 and 25.
  • Ability to greet customers through the medium of Welsh.
  • Enhanced Children and Adults Barred List criminal records check by the Disclosure & Barring Service (DBS).
  • Valid driving licence.
  • Commitment to a person-centred approach and genuine interest in working with young people.

  • Receive a robust induction, regular supervision, additional support and mentoring where required.
