37 hours per week. Join our Integrated Cluster Network Teams, which are dedicated to supporting adults with long‑term complex challenges related to physical, mental, and emotional health. The teams provide holistic care across the life course, ensuring independence, choice, and control for individuals and their families. They work closely with GP surgeries and third‑sector partner agencies to assess and meet the needs of patients and carers.

Responsibilities

  • Promote and deliver high standards of professional practice and intervention within the Integrated Cluster Network Team.
  • Provide leadership to the social work team and support the Social Work Team Manager.
  • Oversee interventions in a dynamic, multi‑disciplinary setting, encouraging innovation and adherence to legislative frameworks.
  • Inspire and mentor team members to achieve the highest level of practice skills and development.

Requirements

  • Fluent in Welsh for customer communication.
  • Enhanced DBS check with Adults Barred list clearance.
  • Valid driving licence.
  • Commitment to the council’s Hybrid Working Policy, balancing home and office hours.
  • Understanding and commitment to child, young person, and adult protection responsibilities.
