- What you’ll be doing
- Responsible to the person in charge for high standards of care to residents in accordance with care plans and appropriate handovers to ensure continuous needs are met.
- Help ensure junior care and ancillary staff contribute to efficient running of the Home and the creation of a homely atmosphere.
- Report any ill health among clients and request GP/Professional visits where necessary.
- Ensure that Clients Care Plans are completed and maintained in conjunction with Clients, relatives and other Health Care Professionals, in accordance with NMC guidelines and company policy.
- Assess all aspects of Clients’ care needs and provide health supervision and direct nursing care.
- Assess social and health care needs of new Clients and maintain ongoing assessment and review of other clients.
- Maintain accurate records and ensure that each Client within the Care Home has an individualised Care Plan. Review Care Plans on a bi-annual basis or more frequently as required.
- Carry out regular checks on Clients at intervals determined by the Home Manager.
Ensure a clear and concise handover report is given to all relevant staff at the end of each shift.
- Undertake tasks including distribution of prescribed medication, dressings, catheterisation, tube feeding, and appropriate intervention in the management of chronic medical conditions; document the same in accordance with company procedures and NMC guidelines.
- Liaise with health and social care professionals involved in the well-being of individual residents to maintain a holistic approach to their care.
- Practice maximum integrity in all dealings with Clients’ personal and financial affairs, and avoid abuse of the privileged relationship that exists with Clients.
- Act at all times within NMC Code of Conduct.
- Maintain criteria for Revalidation, supporting a professional profile in accordance with NMC guidelines.
