The role of Mental Health and Wellbeing Coach will work within the team based across Monkseaton Medical Centre and Bridge Medical. The role will support individuals to move from surviving to thriving by helping people to feel in control of their health and wellbeing by addressing mental health, general health, wellbeing, and inclusion needs using a behaviour change approach. It will span all ages incorporating typical presentations in both adult, child, and adolescent mental health. The postholder will help shape and improve the delivery of contemporary and holistic health support and treatment for Bridge and Monkseaton Medical Practice.
Main duties of the job:
- To provide individual support for people within primary care to explore ways of improving mental health and wellbeing.
- Support people to utilise self-help and facilitated tools to aid individual goal planning and measurement of recovery.
- Support people to make informed choices about their health and wellbeing, which includes improving the levels of physical activity, promoting a healthier diet, and improving mental wellbeing.
- Support access to wider community activities and support as part of an individual plan.
- Effectively signpost and support access to appropriate services.
- Organise group consultations to explore ways of improving mental health and well-being for common conditions; anxiety, low mood, building confidence, carer support etc.
- Contribute to developing leaflets and resources about mental health and wellbeing for patients.
- Utilise social prescribing to develop individual recovery and support planning including working with the team directly across the PCN.
- Promote effective and efficient working practices across the team to achieve measurable outcomes.
- Contribute to, and advise on, the development and implementation of local mental health & wellbeing improvement programmes.
- To share good practice within the PCN and other PCNs in the area and adopt innovative practice where it improves outcome measures for patients and primary care.
About us:
Bridge and Monkseaton Medical Practice are committed to the well being and the development of the whole team. We have a strong focus on staff wellbeing and during the pandemic we created a small garden and zen room to take time out in. We have regular and one off social and wellbeing events such as walking club, sea swimmers, knit and natter, yoga classes, facials and massages.
We are a 2 site merged Practice with Monkseaton Medical Centre and Bridge Medical both located close to the North East coast and excellent bus/metro links to Newcastle upon Tyne. The Practice has excellent facilities at both sites for patients and staff, including meeting rooms and communal kitchen for lunch/coffee. Our team is made up of doctors, paramedic, nurse, Health Care Assistants, pharmacists, pharmacy technician, mental health team, management team and a friendly reception team. We host Doctors in training, Foundation doctors and 5th year medical students.
Job responsibilities:
- Provide individual support for people within primary care to explore ways of improving mental health and wellbeing.

- Support primary care colleagues to address the wider determinants of health and deliver individual package of support and education.
- Organise group consultations to explore ways of improving mental health and well-being for common conditions; anxiety, low mood, building confidence, carer support etc.
- Contribute to developing leaflets and resources about mental health and wellbeing for patients.
- Communicates sensitively and effectively with clients, and their relatives and carers, including those with special needs and those who do not comprehend written and spoken English, to promote changes in behaviour that lead to health benefits.
- Communicates tactfully and uses persuasive skills with clients where there are barriers to understanding.
- To communicate to clients and staff to promote learning and behaviour change.
- Sensitively communicates with patients, their relatives and carers when dealing with difficult issues raised by patients, including disclosure of abuse.
- Clearly communicates to staff and to line manager/GP to enable the service to be delivered effectively and efficiently.

Experience:
- Experience of involving patients and local communities in identifying their own health needs.
- Experience of working in a multi-agency environment.
- Experience on supervising staff.
- Experience of using clinical systems such as SystmOne.
- Experience of supporting people with their mental health.
- Experience of using tools to support people to have a shared decision-making conversation.
Qualifications:
- Knowledge of public health/health improvement, acquired through training, extended courses and experience to degree level equivalent.
- Health coaching/counselling qualification or willingness to undertake this study.
- Experience of working in a multi-agency environment.
- Knowledge of the issues relating to topic area.
- Knowledge of local resources available to help support these patient groups.
- Teaching/Training experience.
