Job purpose: The post holder will be an integral part of the practice team. A referral to a non-medical link worker is designed to support patients in taking a holistic approach, connecting people to community groups and statutory services for practical and emotional support. Social prescribing can help to strengthen community resilience and personal resilience whilst reducing health inequalities by addressing wider determinants of health such as debt, poor housing, and physical inactivity. This role can be particularly beneficial to patients with long-term conditions, mental health issues, loneliness, social needs, or complex social circumstances affecting wellbeing.
Primary key responsibilities: The core responsibilities of the Social Prescriber Link Worker include assessing patients\’ needs, co-producing care plans, evaluating progress, providing personalized support, developing trusting relationships, and taking a holistic approach based on patient priorities. The role involves supporting access to employment, training, welfare, managing caseloads, working with multidisciplinary teams, building community relationships, inputting referral data, raising awareness, encouraging self-referrals, and offering home visits when appropriate. The post also involves supporting individuals to identify wider issues impacting health, maintaining independence, exploring personal health budgets, and working with local organizations and partners to address unmet needs and develop community services.
