The Post Placement Relationship Officer – Community and Prevention Team will shape and enhance relationships with existing community-based providers in Brent, driving improvements across the sector.
Key Responsibilities:- Support commissioning of community services that meet required contract standards and outcomes.
- Undertake contract monitoring with designated providers, identify quality concerns, and work with providers to develop, implement and monitor quality improvement plans.
- Understand supply and demand in the market and help grow and shape services accordingly.
- In-depth understanding of adult social care and a passion for improving service quality and sustainability in Brent.
- Experience in commissioning care services, preferably within adult social care.
- Proven stakeholder engagement, especially with community-based providers.
- Strong quality assurance skills.
- Ability to manage multiple workstreams while focusing on outcomes.
- Effective communication and influencing skills.
- Up to 33 days annual leave (depending on length of service) plus bank holidays.
