Are you passionate about supporting vulnerable people with complex needs and helping individuals move away from homelessness and substance misuse? We are looking for a compassionate and proactive Complex Lives Navigator to join a specialist Rough Sleeper Outreach Service in Weymouth. This is a rewarding opportunity to work directly with people experiencing rough sleeping, providing trauma-informed support and specialist interventions to help individuals achieve stability, recovery, and long-term independence.

The Role

As a Complex Lives Navigator, you will work as part of an established outreach team supporting people with a range of complex needs, including substance misuse, mental ill-health, domestic abuse, and behavioural challenges. You will build trusting relationships with individuals who may be disengaged from services and support them in accessing housing pathways, healthcare, and specialist support. This role includes participation in outreach services during early mornings, evenings, and weekends, as well as involvement in an out-of-hours on-call rota.

Key Responsibilities

  • Deliver person-centred support to individuals experiencing homelessness and complex needs
  • Provide specialist support relating to substance misuse and recovery pathways
  • Build professional and trusting relationships using a trauma-informed approach
  • Support individuals to access housing, healthcare, and community services
  • Maintain accurate and up-to-date client records and support plans
  • Participate in Multi-Disciplinary Team (MDT) meetings and collaborative partnership working
  • Take part in outreach shifts and out-of-hours on-call support

What We’re Looking For

  • Experience supporting individuals with complex needs and/or challenging behaviours
  • Knowledge or experience relating to homelessness, substance misuse, mental health, or outreach services
  • Strong communication and relationship-building skills
  • Ability to work independently and as part of a multidisciplinary team
  • Compassionate, resilient, and proactive approach
  • Full UK driving licence and access to your own vehicle for work purposes
  • Business insurance will be required for work-related travel

Requirements:

  • Must have full driving licence and access to own vehicle.
  • Experience working with individuals with complex needs and/or challenging behaviours.
  • Required to participate in out-of-hours on-call rota.
  • Required to participate in outreach rota which will include early morning, evenings, and weekends.
