BRC is recruiting a Case Worker to join a dedicated and supportive team delivering community‑based dementia support across Cardiff. This is a fixed‑term opportunity offering a rewarding and varied role where no two days are the same. It allows you to support individuals living with dementia to remain active, achieve personal goals, maintain independence, and improve their overall well‑being.
Key responsibilities:
- Provide one‑to‑one support to individuals living with dementia
- Complete assessments and develop person‑centred support plans
- Support individuals to achieve meaningful goals and activities
- Facilitate group activities within community and care settings
- Provide practical and emotional support to families and carers
- Work alongside volunteers, health professionals and partner agencies
- Maintain accurate records and case‑management systems
Experience and skills:
- Experience within health and social care, support work or community services
- Desirable: Experience supporting individuals with dementia
- Ability to work independently and manage a varied caseload
- Strong communication and relationship‑building skills
- Full UK driving licence and access to own vehicle
- Good IT and administration skills
