CARE COODINATOR WARWICK We are looking for a dedicated and experienced Care Coordinator to join our client’s management team and help maintain their high standards. This person MUST have a UK drivers licence and own car full time and MUST have the Right to Work in the UK, THERE IS NO VISA SPONSORSHIP IN PLACE AT THIS COMPANY. The Care Coordinator will be a key role in the team, ensuring that care services are provided in line with individual care plans, service agreements, and the required standards. This is a hands-on role, involving the supervision of care teams, assessing client needs, and co-ordinating the delivery of care services. The successful candidate will be responsible for liaising between clients, staff, and management to ensure the seamless provision of care. BENEFITS Salary – £28000 – £30000 – dependent on experience 20 days holidays + bank holidays Pension after 3 months Lots of other benefits ie: blue card, etc. DUTIES Conduct assessments and develop individual care plans for clients. Ensure continuity of care by managing and implementing an organised rota system. Supervise care workers and teams, being first point of contact and offering guidance, advice, and mentoring. Provide direct care to service users in emergency situations or out of hours. Conduct spot checks on care delivery and report findings to management. Regularly visit service users to gather feedback and address any concerns or issues. Investigate and report on concerns, including safeguarding issues and complaints, to the service manager. Assist with staff induction and training and assess new staff for their ability to work independently. Review daily notes, incidents, and concerns to ensure high-quality care and investigate any complaints. Provide support to the senior management team, including the Registered Manager (RM). Keep up to date with company policies, procedures, and relevant industry guidelines. Collaborate with other professionals involved in the care of clients to ensure comprehensive support. Contribute to the service’s quality improvement and business planning strategies. Engage in continuous professional development through relevant training and qualifications. Ensure compliance with the Health and Care Act 2012, and CQC 5 Domains: Safe, Caring, Effective, Responsive, and Well-led. Follow national guidelines such as NICE guidelines and Skills for Care. EXPERIENCE NEEDED Previous experience in a care coordination or related care role is essential, preferably in Domiciliary care. Relevant qualification (Certificate/Diploma) at an appropriate level – Level 2/3 or above. Strong leadership and supervisory skills. Excellent communication, interpersonal, and organisational skills. Ability to work flexibly and respond effectively to emergencies. Commitment to continuous professional development. Knowledge of the Health and Care Act 2012, CQC standards, and national guidelines
