MAIN PURPOSE
To support the Therapy staff to provide high quality holistic person-centred care and support to service users to reach their full rehabilitation potential.
MAIN TASKS AND RESPONSIBILITIES
- Together with the multi-disciplinary team responsible to ensure optimum cognitive, social, emotional, physical and functional needs are addressed.
- To support service users with cancer or other life-limiting illnesses, who have specialist palliative or neurological needs.
- Responsible to ensure that the service users’ individual rights and choices are actively promoted and supported, and their beliefs and preferences appropriately acknowledged.
- Responsible to report status and variances in treatment and condition to the appropriate Therapist at the earliest opportunity and progress treatment as advised by the Therapist.
- Where appropriate, responsible to review, order and fit equipment and work with service users and their carers/family to demonstrate its safe use, either in the centre or in service user’s home.
- Responsible to liaise with service users and family members to help complete forms, gather information and co-ordinate interventions.
- To support the Therapy Team by completing administrative tasks such as gathering referrals, recording monthly activity data.
- Responsible to collect, collate, evaluate and report information, maintaining accurate service user records in all formats, including written and electronic information systems.
- Facilitating the safe and smooth running of the Service, the post holder is expected to maintain a safe working environment, maintain service user confidentiality, and occasionally taking on reasonable additional tasks.
