We are looking for a Clinical Pharmacist to join our supportive and forward-thinking team in a beautiful part of Hampshire. We are happy to consider part-time or full-time applicants, with salary negotiable based on skills and experience.
You will be joining a practice with a strong history of mentorship and professional development, where collaboration and colleague support are genuinely valued.
About the Practice
We are a friendly, mutually supportive GP Partnership consisting of two Partners and thirteen Associate GPs, serving a patient population of approximately 17,250 patients. Our well-established multi-disciplinary team includes:
- Advanced Nurse Practitioners (ANPs)
- Triage Nurses
- Treatment Room Team
- Additional PCN roles supporting daily service delivery
Main Duties and Responsibilities
As a Clinical Pharmacist, you will play a key patient-facing role within the multi-disciplinary team. Core responsibilities include:
- Clinically assessing and treating patients using expert knowledge of medicines for specific disease areas
- Managing care for patients with chronic conditions, including undertaking Structured Medication Reviews (SMRs)
- Proactively managing patients with complex polypharmacy, particularly:
- Elderly patients
- Care home residents
- Patients with multiple co-morbidities (including frailty, COPD, and asthma)
- Patients with learning disabilities or autism (via the STOMP – Stop Over Medication Programme)
