An exciting opportunity has arisen for an experienced Practice Nurse to join a well-established, patient-focused GP practice serving approximately 10,500 patients in Halifax. This forward-thinking practice is seeking a motivated and enthusiastic nurse with previous General Practice experience to join its friendly and supportive clinical team. The successful candidate will play a key role in delivering high-quality patient care while contributing to the ongoing development of services within the practice. - Chronic Disease Management (Diabetes, Asthma & COPD)
- Cervical Cytology (Smears)
- Childhood & Adult Immunisations
- Wound Care & Dressings
- Contraceptive Advice & Support
- NHS Health Checks & Screening Programmes
- Health Promotion & Lifestyle Advice
- Infection Prevention & Control
- Supporting the Practice in achieving QOF, IIF and Quality Targets
- Supporting the development of Nurses and Healthcare Assistants where appropriate

Essential Requirements
- Registered Nurse (NMC Registered)
- Previous experience working as a Practice Nurse within a GP Surgery
- Experience managing long-term conditions such as Diabetes, Asthma and COPD
- Competent in Cervical Cytology (Smears)
- Experience delivering Childhood & Adult Immunisations
- Strong communication and teamwork skills
- Evidence of ongoing professional development
- Competent using SystmOne
Desirable Skills
- Independent Nurse Prescriber Qualification
- Additional qualifications in Chronic Disease Management
- Experience in Minor Illness Clinics
- Triage Experience
