Job summary

The post-holder is an Advanced Clinical Practitioner (ACP).

They have been assessed as competent in practice using their advanced clinical knowledge and skills.

They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.

The post-holder works at the level of advanced clinical practice and has developed and can evidence the underpinning competencies applicable to the specialty or subject area, i. e., the knowledge, skills and behaviours appropriate to the healthcare professionals setting and job role.

ACPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health.

An ACP is autonomous in making decisions based on assessment, diagnosis and the interpretation of test results and will initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care.

Main duties of the job

• Clinical responsibilities
• To independently undertake clinical assessments, diagnose, treat and discharge, or appropriately refer, patients with a wide range of acute and chronic illnesses.
• To ensure patients are fully involved in the design and implementation of their treatment plans.
• To ensure effective coordination and liaison with multidisciplinary teams across primary and secondary care, statutory and voluntary disciplines/groups to provide a seamless service to patients and their carers and prevent hospital admission where possible.
• To refer appropriately to clinical leads/supervising GP where a second opinion is required.
• To adapt styles of communication appropriately to promote empowerment, self management and health education for patients.
• To attend clinical case conferences, review and clinical meetings as required.
• To be a source of clinical expertise to less experienced staff within the team.
• To delegate duties/tasks to other members of staff where appropriate.
• Provide specialist advice relating to minor illness and/or injury to local healthcare teams, patients, relative, and carers.
• To identify actual or potential clinical risks and take appropriate action using, when required, the appropriate reporting structure.
• Able to interpret and file pathology results
• Manage patients across all age groups, including children.

About us

We are a University Practice, seeking to appoint an Advanced Clinical Practitioner to join our team.

Bradford Student Health Service is a busy GP practice in Bradford with approximately 8,500 registered patients.

We are a university practice situated near to the University of Bradford.

We are a member of PCN 6.

We are looking for applicants who are energetic, enthusiastic and focused on delivering a high standard of care to our patients.

You should be proficient in all aspects of general practice nurse duties.

Our clinical team consists of two GP partners, locum GP's, a Practice Nurse, a Health Care Assistant, two GP Assistants, three Clinical Pharmacists and a Pharmacy Technician.

The clinical team are supported by a Practice Manager, Operations Manager, Administrators and a Medical Secretary.

Our reception team work as Care Navigators providing enhanced signposting and ensuring patients have access to the appropriate clinicians or services available to them.

The practice is a consistently high performer in relation to its contractual obligations including QOF and Quality Contract services.

• Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
• Maintain accurate clinical records in conjunction with extant legislation ensuring that SNOMED CT codes are used effectively
• Maintain chronic disease registers and implement an effective call/recall system
• Chaperone patients where necessary
• Prioritise health issues and intervene appropriately
• Support the team in dealing with clinical emergencies
• Recognise, assess and refer patients presenting with mental health needs
• Support in the provision of vaccination programmes for both adults and children
• Be an extended and supplementary prescriber, adhering to extant guidance
• Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

Qualifications

Essential

• Qualified healthcare professional that is educated to Masters degree as detailed within the HEE document tilted Multi-professional framework for advanced clinical practice in England
• Has evidence of working at an enhanced level, as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework
• Proof of relevant professional and educational qualifications, coupled with details of any training
• Registration with their professional governing body
• Qualified independent prescriber
