Job summary

The post-holder is an Advanced Clinical Practitioner (ACP).

They have been assessed as competent in practice using their advanced clinical knowledge and skills.

They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.

The post-holder works at the level of advanced clinical practice and has developed and can evidence the underpinning competencies applicable to the specialty or subject area, i. e., the knowledge, skills and behaviours appropriate to the healthcare professionals setting and job role.

ACPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health.

An ACP is autonomous in making decisions based on assessment, diagnosis and the interpretation of test results and will initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care.

Main duties of the job

  • Clinical responsibilities
  • To independently undertake clinical assessments, diagnose, treat and discharge, or appropriately refer, patients with a wide range of acute and chronic illnesses.
  • To ensure patients are fully involved in the design and implementation of their treatment plans.
  • To ensure effective coordination and liaison with multidisciplinary teams across primary and secondary care, statutory and voluntary disciplines/groups to provide a seamless service to patients and their carers and prevent hospital admission where possible.
  • To refer appropriately to clinical leads/supervising GP where a second opinion is required.
  • To adapt styles of communication appropriately to promote empowerment, self management and health education for patients.
  • To attend clinical case conferences, review and clinical meetings as required.
  • To be a source of clinical expertise to less experienced staff within the team.
  • To delegate duties/tasks to other members of staff where appropriate.
  • Provide specialist advice relating to minor illness and/or injury to local healthcare teams, patients, relative, and carers.
  • To identify actual or potential clinical risks and take appropriate action using, when required, the appropriate reporting structure.
  • Able to interpret and file pathology results
  • Manage patients across all age groups, including children.
  • No restrictions on prescribing.
  • Able to see pregnant women in regards non pregnancy related issues
