Bradford Corporation is seeking a dedicated Reablement Coordinator for the Bradford Reablement Service. This role involves assessing and implementing care packages to support individuals in the community, working every Saturday and Sunday for 15 hours each week.

The successful candidate will need to have minimum Level 2 in Health and Social Care along with significant experience in adult care. This position offers a unique opportunity to work closely with individuals and their families to enable them to achieve their care goals.
