We are looking to recruit an enthusiastic Reablement Coordinator to join the Bradford Reablement Service working for Bradford Metropolitan District Council in the community within the Bradford districts.

Responsibilities

  • Assess needs and implement (and review) care packages that meet the Care Act 2014.
  • Work with individuals and their families to support people to reach their full potential through self‑care and reablement.
  • Complete and review reablement support plans.
  • Supervise and support a team of reablement support workers to ensure the best service is delivered in accordance with all relevant departmental policies and guidelines to enable compliance with the Care Quality Commission (CQC).
  • Carry out performance management including supervisions, appraisals, and observations.
  • Plan and lead team meetings.
  • Support and mentor trainee reablement support workers to achieve the care certificate and Level 2 in health and social care.
  • Participate in flexible and integrated working with other partners.
  • Work 15 hours per week, every Saturday and Sunday between 7:00 and 22:00.

Qualifications

  • QAF diploma (NVQ/QCF) at minimum Level 2 in Health and Social Care and a willingness to achieve Level 3.
  • A full UK driving licence.
  • Significant (and demonstrable) experience of working in adult care.
