As the PCN Service Coordinator, you will support the delivery of services across our Primary Care Practices, health promotion and work with stakeholders across the health and care system to deliver projects. The post holder is expected to work accurately and under pressure with the ability to manage and prioritise their own workload, that of the department and anticipate and resolve problems expediently.

Main duties of the job:
  • Compile and disseminate rotas for the clinical and non-clinical staff and act as a single point of contact.
  • Ensure staff comply with all company policies, procedures and guidelines.
  • Ensure that services are staffed appropriately.
  • Co-ordinate rota changes at all levels to ensure minimal impact on care and be the first point of contact for all day-to-day issues affecting staff cover.
  • Support the rollout of new initiatives/changes and services and cascade key information and deal with enquiries, escalating where necessary.
  • Coordinate and promote events such as Health Check events and pre-diabetes screening.
  • Manage your own workload, use own initiative, communicate effectively, and escalate issues as appropriate.
  • Support the Operations and Business Manager in the delivery of local projects.
About us:

Bradford City Primary Care Network 4 (PCN4) is a network of Bradford City Practices working collaboratively across the network area to look after the health of our local population. The PCN comprises of 5 local Practices with a total population of around 44,000 registered patients.

Job responsibilities:
  • Day-to-day operational responsibility for the applying and maintenance of rotas, coordinate locum cover when required.
  • Check all locum timesheets to ensure agreed rates and hours are charged and any discrepancies referred to the relevant personnel.
  • Coordinate events to promote health screening and work with external stakeholders to deliver local initiatives.
  • Support the set up and mobilisation of new services.
  • Monitor progress against the delivery of new services by tracking actions and producing project updates.
  • Produce reports (as required) on activity, compliance and procedures.
  • Continually assess and evaluate systems recommending changes and improvements.
  • Act as a liaison between PCN practices, community services, voluntary sector organisations and external partners to support service delivery.
  • Undertake mandatory training and keep staff training records up to date.
  • Work in accordance with Health and Safety regulations at all times.
  • Work at any location where the PCN provides services.
  • Abide by all information governance requirements in respect of data protection and confidentiality.

  • Experience in SystmOne rota design and implementation.

  • Experience of implementing organisational policies and maintaining associated documents.
  • Experience of working with clinical staff.
  • Previous experience within Primary care is desirable, but a willingness to learn, transferable experience, and an empathetic leadership approach are more important.
Disclosure and Barring Service Check:

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

£31,000 to £35,000 a year pro-rata depending on experience.
