This role is based at Bradford Teaching Hospitals NHS Foundation Trust(BTHFT), covering the Urgent Treatment Centre (UTC) and Emergency Department.The service operates between 08:00 and 00:00, 7 days per week, and shifts (6 or4 hours) will be allocated on a rota basis. Flexibility, including weekends andevenings, is required to meet service needs.

Various shifts and hours available, successful candidates must commit to at least 1 x 6 or 4-hour shift a week as a minimum.

Main duties of the job

Healthcare Assistants(HCAs) working in the Primary Care Streaming Service at the UTC and EmergencyDepartment play a vital role in patient flow, clinical safety, and serviceefficiency. The role includes reception support, patient observations,point-of-care testing, and communication with clinical teams. HCAs are expectedto maintain high standards of professionalism, safety, and confidentiality inline with BCA and BTHFT policies.

Thepost-holder will provide direct support to clinicians and patients byperforming clinical observations, assisting with point-of-care testing, andensuring smooth patient streaming processes. The HCA will monitor patientsafety, act as a chaperone when required, and liaise with both UTC and A&Eteams. They will also play an active role in ensuring efficient communicationacross the department and adherence to safety and data protection protocols.

Job responsibilities

Receive patients and guide them through thestreaming process.

Collect and escort patients to the UTC asrequired.

Provide a professional, welcoming, andreassuring presence.

Act as a chaperone when necessary.

Observations & ClinicalSupport

Completebaseline observations within 30 minutes of registration and repeat asrequired

Carry outpaediatric observations every 4 hours.

Monitorwaiting patients for safety and escalate concerns appropriately.

Escalateclinical concerns to the GP on site, Sister in Charge, or UTC Lead Nurse.

Record andclose patient cases accurately in electronic systems

Point-of-Care Testing(POCT)

Undertakeapproved point-of-care tests in line with training.

Recordresults promptly in patient records.

Liaise withclinical teams if tests indicate the need for admission.

Patient Safety &Protocol Adherence

Adhere to patient influx protocol when demandexceeds safe levels.

Ensure no patient is left without observations formore than one hour.

If leaving the department without HCA cover, pausestreaming and notify GP/Admin team.

Support admin team to ensure reception cover duringbreaks.

Teamwork &Communication

Work effectively with colleagues to ensuretimely patient care.

Share patient responsibility clearly when morethan one HCA is on duty.

Use Cerner initials to identify accountable HCAfor each patient.

Maintain visibility in reception and waitingareas to support workflow.

Information Governance & Data Protection

Useindividual logins for SystmOne and Cerner only.

Recordobservations and POCT under your own name.

Lockcomputers when unattended and never share passwords.

Adhereto BCA and BTHFT information governance and data protection policies.

Restrictmobile phone use to emergencies only.

Training & Development

Participatein training to support ongoing development.

Remainup to date with SOPs and trust policies.

Engagein safety training and clinical updates.

Contributeto a culture of continuous learning and improvement.

Person Specification

Experience

  • Experience as a Healthcare Assistant or equivalent patient-facing role.
  • Experience in urgent/emergency/primary care settings.

Personal Qualities

  • Confidential, discreet, proactive, and able to work under pressure.
  • Ability to use initiative and willingness to take on additional tasks.

Skills

  • Ability to perform and record patient observations accurately.
  • Strong communication and interpersonal skills.
  • Strong attention to detail.

  • POCT training and experience.
