We are looking for a dedicated Social Worker to join our Community Team for People with Learning Disabilities (CTPLD) and Autism. This is a full-time, permanent position that supports adults with learning disabilities and autism through assessment, support planning, direct work, and general case management.
Responsibilities
- Hold a caseload of individuals with learning disabilities and autism and conduct statutory duties on behalf of the service.
- Carry out assessment, support planning, review and direct work with clients.
- Provide general case management, including collaboration with other agencies and services.
Qualifications
- Demonstrated experience working within the current Social Care System and Framework.
- Registration with Social Work England (or eligibility for registration).
- Ability to work within a hybrid working model and attend the Time Square office at least two days per week.
