About the Role
We are looking for a dedicated Social Worker to join our Adult Social Care Hospital Team. We work with some of our most vulnerable residents using a compassionate, strength‑based approach that keeps each person firmly at the centre of their care.

Team and Working Environment
Our Adult Social Care service is made up of highly professional multidisciplinary teams that work closely with health and other partners to prevent unnecessary hospital admissions and support people to return home safely. Regular supervision and access to training are provided to support staff development.
Key Responsibilities
- Manage a caseload, organising and prioritising the workload effectively, with regular supervision and guidance.
- Work as part of a multidisciplinary team to connect people and their families to community and social networks, enabling them to live within their own communities.
- Collaborate with colleagues to ensure people receive the right care at the right time in the right place.
- Take a strengths‑based approach to support people to live the best life they can and reduce dependence on long‑term care.
- Support the delivery and maintenance of high standards of performance across the service.
Qualifications and Experience
- Social Work England registered.
- At least 2 years post‑qualification experience.
- Demonstrated experience of working within the current Social Care System and Framework.
