You will work closely with colleagues across Adult Social Care, housing partners, contractors and Housing Associations, contributing to creative, person‑centred solutions that make a real and lasting difference to residents’ independence and wellbeing.
Main Duties of the Job
- Undertake person‑centred assessments to identify housing adaptations that promote safety, independence and long‑term wellbeing
- Work collaboratively with service users, colleagues and partners to develop and progress DFG solutions
- Contribute specialist OT knowledge to multidisciplinary meetings, case discussions and service development
- Produce clear, evidence‑based reports and recommendations to support DFG applications and adaptation specifications
- Maintain accurate electronic records in line with professional and organisational standards
- Engage fully in supervision, appraisal and ongoing professional development
- Practice in accordance with council policies and the HCPC Code of Conduct and Standards of Proficiency
