Join our team to deliver high-quality, same-day care to our patients in the Northamptonshire Rural East & South (NRES) Neighbourhood. We are looking for an experienced Advanced Nurse Practitioner to join our Neighbourhood Access Service, part of the innovative New Models Programme. You will work autonomously and as part of a multidisciplinary team within primary care to provide expert assessment, diagnosis, treatment, and evaluation for patients presenting with minor illness, acute conditions, and common general practice presentations, including women's health. Main duties of the job:
- Provide autonomous care for patients presenting with minor/acute illness and common general practice presentations, including those relating to women's health.
- Demonstrate safe clinical decision making and provide expert care by undertaking initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.
- Work with GPs and nursing team in the Practice to continually review and evaluate services and work with the management team in implementation of change.

About us:
The Northamptonshire Rural East & South (NRES) Neighbourhood, consisting of 7 GP Practices serving 88,938 patients, is developing an innovative new model of care as part of the NHS New Models Programme. You will be working with a team of GPs and specialist ANPs to deliver same day care to the Rural East & South population across our three hub sites.
Job responsibilities:
- Provide clinical care to patients, reducing the demand on GP time by assessing and managing patients through telephone triage and clinic appointments.
- Use advanced skills and expert knowledge to assess the physical and psychosocial needs of patients with common primary care conditions and minor illness, instigating and providing therapeutic treatments based on best available evidence to improve health outcomes.
- Promote patient independence and self-care where appropriate.
- Undertake the role of independent non-medical prescriber in the treatment and care of patients.
- Review pathology results to ensure appropriate treatment and care.
- Support and lead the practice for the delivery of the Quality and Outcomes Framework and NHS England/ICB/Federation Contracts.
Regulatory:
- Respond to complaints and clinical incidents/significant events as appropriate to ensure robust systems are in place for improving practice through joint learning.
- Undertake at least one clinical audit a year, evaluate data, and present findings to the Practice Clinical team.
- Adhere to the Practice Policies and Procedures including Infection Control, Health and Safety.

Personal / Professional Development:
- Maintain own personal/professional development plan and portfolio thereby meeting the requirements for revalidation through appraisal of own performance.
- Maintain NMC registration and professional development in line with expectations of the role.
- Provide mentorship and training of new recruits or students as and when required.
- Participate in the Practice Performance Review process.

Person Specification:
Experience:
- In-depth knowledge base in the management of patients with chronic illness and minor illness.
- Ability to maintain and monitor high standards of evidence-based care.
- Ability to direct and co-ordinate care working autonomously and collaboratively.
- Understanding of clinical governance issues.
- Evidence of improving the patient's experience.
- Evidence of leading a service project at practice level.
- Undertaking research or involvement in clinical research programmes.
- Experience in writing clinical policies and protocols.
- Counselling experience.
Qualifications:
- RGN with a minimum of 5 years post-registration experience.
- Independent non-medical prescriber.
- Community/practice nurse qualification or equivalent experience.
- Extensive clinical experience in primary nursing.
- Evidence of continuing professional development.
- Post-registration qualification in minor illness/injury, women's health/mental health/paediatrics.
- Teaching qualification.
- Recent attendance on a leadership skills development programme.
