We are working closely with a local authority in Wirral MBC to assist with the appointment of a Placement & Brokerage Officer, on a 3-month contract, highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Rate of Pay: £12.81- £16.33 per hour

Summary:

The Placement & Brokerage Officer plays a key role within Wirral MBC's Care Arrangement Team, coordinating and securing care packages and placements for adults with assessed care and support needs. Working within Adult Care and Health, the post holder acts as the link between Adult Social Care practitioners, health professionals, care providers, and service users to ensure individuals receive timely, appropriate, and cost-effective support.

Responsibilities:

  • Coordinate domiciliary care, residential care, nursing placements, respite care, and specialist support services.
  • Develop and maintain effective relationships with commissioned care providers.
  • Ensure referrals contain appropriate information to support effective placement decisions.
  • Ensure services are commissioned in accordance with approved financial frameworks.
  • Ensure placements reflect assessed needs and individual preferences where possible.
  • Support continuity of care during transitions between services.

Qualifications:

Educated to GCSE level or equivalent.

Essentials:

  • Experience working within an Adult Social Care environment.
  • Experience maintaining accurate records and documentation.
  • Experience managing competing priorities and meeting deadlines.
