We are working closely alongside a local authority in Southwark to assist with the appointment of an Independent Living Facilitator on a 4-months contract, highly likely to be extended at the client's discretion. Please apply with your CV for immediate consideration.
As an Independent Living Facilitator, you will join a dynamic, person-centred team within Southwark's Adult Social Care division. Your mission is to provide strength-based, time-limited interventions that help young people with learning disabilities, autism, and sensory or physical needs achieve the outcomes listed in their Education, Health and Care Plans (EHCP). You will act as a catalyst for independence, helping individuals build the confidence to navigate their communities and lead fulfilling, self-directed lives.
Responsibilities:
- Deliver tailored support in line with EHCP outcomes, focusing on maximizing independence and resilience through interventions at the Southwark Resource Centre and in the community.
- Act as a keyworker to identify and facilitate meaningful goals.
- Develop person-centred support plans and assist individuals in setting and reaching milestones.
- Provide hands-on, person-centred support for individuals with complex needs, including developing communication skills and assisting with physical needs (toileting, mobility, eating) with the utmost dignity and respect.
- Conduct risk assessments and implement safety plans.
- Work within a Positive Behaviour Support (PBS) framework to manage behaviours of concern safely and effectively.
Qualifications:
- NVQ Level 3 in Health and Social Care (or equivalent), or a demonstrated willingness to work towards this qualification.
Essentials:
- Proven experience working with people with disabilities in a paid or voluntary capacity.
- Experience in developing outcome-focused plans and assisting individuals with goal setting.
- A deep understanding of the social, physical, and emotional needs of young people with disabilities, including various communication methods.
- Robust knowledge of safeguarding procedures and a commitment to supporting vulnerable persons to remain safe.
Please note: You should be available to work immediately or at short notice. You should have the right to work in the U.K. This role requires an Enhanced DBS.
