We are working closely with the local authority in Slough to assist with the appointment of a Continuing Healthcare Lead, on a 3-month contract, highly likely to be extended at the client's discretion. Please apply with your CV for immediate consideration. Rate of Pay: £221.24- £299.00 per day.
The Continuing Healthcare (CHC) Lead will act as the subject matter expert in Adult Continuing Healthcare, providing specialist professional leadership, operational guidance, and strategic support to Adult Social Care services. The post holder will ensure that Continuing Healthcare practice is delivered in accordance with national legislation, statutory guidance, local policies, and best practice frameworks, while supporting lawful, defensible, and person-centred decision-making.
Responsibilities:
- Act as the Council's subject matter expert on Adult Continuing Healthcare (CHC), providing specialist advice, professional leadership, and operational guidance across Adult Social Care.
- Ensure care and support plans relating to CHC-funded care are reviewed in line with departmental procedures, contractual requirements, and best practice standards.
- Represent the Local Authority in disputes relating to Continuing Healthcare funding decisions, including disputes regarding CHC refusals.
- Identify training needs and support continuous professional development relating to Continuing Healthcare practice.
- Support responses to complaints, Freedom of Information (FOI) requests, Data Protection Act (DPA) enquiries, and governance processes.
Qualifications:
- Professional qualification in Social Work, Nursing, Occupational Therapy, Health or Social Care, or a related professional discipline.
- Registration with the relevant professional body where applicable (e.g., Social Work England, Nursing and Midwifery Council, or equivalent).
- Specialist knowledge or training in NHS Continuing Healthcare frameworks, Decision Support Tools (DSTs), and eligibility processes is essential.
Essentials:
- Knowledge of the Care Act 2014, Health and Social Care Act 2012, Mental Capacity Act, safeguarding, and adult social care legislation is essential.
- Significant experience working within Adult Social Care, Continuing Healthcare, health, or integrated care settings.
- Extensive experience completing Continuing Healthcare Checklists and Decision Support Tools (DSTs) and supporting CHC applications.
- Experience undertaking audits, quality assurance activities, or service improvement relating to professional practice.
