To provide effective brokerage and support services for adults who require short-term or long-term care and support. The postholder will work with individuals, families, social care and health professionals, and care providers to source appropriate care placements and packages in line with assessed care and support needs. The role will contribute to safe and timely hospital discharges, ensuring that people receive person-centred care and support while making effective use of available resources.
Responsibilities:
- Source appropriate residential, nursing, domiciliary and other care and support services for adults based on their assessed needs.
- Support timely and safe hospital discharges by identifying and securing appropriate care and support arrangements.
- Respond effectively to changing priorities and urgent requests, particularly where there are risks to an individual's safety or discharge delays.
