- Lead on the coordination and implementation of individual Pathway Plans for young people transitioning from care.
- Identify and support young people experiencing elevated levels of risk, including poor mental health, anti-social behavior, or the risk of losing employment and housing.
- Ensure all transitions are managed in a "safe and healthy manner," maintaining awareness of both children's and adult service frameworks.A relevant qualification in Social Care, Youth Work, or a related field (typically NVQ Level 3 or equivalent).
- A high standard of general education, including English and Maths.
Essentials:
- Proven experience of working directly with young people (ideally aged 16-25) in a social care, youth work, or community setting.
- Experience in supporting vulnerable individuals through significant life transitions, particularly moving toward independent living.
- Understanding of the legislative frameworks governing both Children's Services and Adult Social Care.
