We are working closely alongside a local authority in Bexley to assist with the appointment of a Social Care Assistant on a 6.5‑month contract, highly likely to be extended at the client’s discretion. Please apply with your CV for immediate consideration.

The Social Care Assistant (Level 1) is a vital frontline role dedicated to supporting adults, older people, and their informal carers. Operating within the framework of the Care Act, you will assess the needs of individuals with physical disabilities, mental health conditions, and learning disabilities. Your primary focus is to implement strengths‑based support plans that maximise independence, allowing service users to remain in their communities or move into appropriate accommodation. This role requires a rehabilitative and preventative approach. You will work as part of a multi‑disciplinary team, collaborating with health colleagues and external partners to ensure a seamless, integrated support system. At the Bexley 06 level, you will manage a caseload of lower complexity under the oversight and guidance of qualified senior practitioners.

Responsibilities

  • Conduct strengths‑based assessments of service users and informal carers to determine eligibility under the Care Act.
  • Formulate and implement individualized support plans that prioritise independence and preferred lifestyle choices.
  • Conduct initial enquiries and risk assessments regarding safeguarding concerns for vulnerable adults (with senior support).
  • Assess daily living activities and organise solutions such as minor home adaptations and assistive equipment.
  • Undertake Continuing Health Care (CHC) checklists as part of a multi‑disciplinary team.

Qualifications

  • NVQ Level II in Health and Social Care (or equivalent practical knowledge).
  • Recent experience (paid or voluntary) working with people with social care needs (Learning Disabilities, Physical Disabilities, Mental Health, or Older Persons).
  • A foundational understanding of the Care Act, Mental Capacity Act, and Equal Opportunities legislation.
  • Awareness of local universal services, welfare benefits, and data protection/confidentiality principles.
