We are working closely with local authority in Liverpool to assist with the appointment of a Reablement Assistant, on a 3-months contract, highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

The Reablement Assistant will provide high-quality, person-centred support to adults requiring assistance to maintain, regain, or improve independence in their own homes and communities. Working within Adult Social Care & Health services, the postholder will support service users with physical, emotional, and social needs in accordance with agreed care and support plans, helping individuals enhance daily living skills and maximise independence.

Responsibilities:
  • Support service users to meet physical, emotional, and social care needs as outlined in agreed care plans and support plans.
  • Assist service users in implementing person-centred support plans focused on independence, confidence, and improved quality of life.
  • Promote high standards of care practice, maintaining dignity, independence, empowerment, privacy, and respect for cultural, spiritual, religious, and individual needs.
  • Observe, monitor, and report significant changes in service users' needs, wellbeing, health, or circumstances to the Line Manager and relevant professionals.
  • Comply with Council policies, procedures, statutory regulations, financial regulations, standing orders, and safe systems of working.

Essentials:

  • Experience supporting adults with physical, emotional, social, or practical support needs in a care, community, support, or health-related setting.
  • Experience assisting individuals to maintain or improve daily living skills and independence.
  • Good understanding of reablement, independence promotion, and person-centred support approaches.
  • Understanding of safeguarding, equality and diversity, confidentiality, anti-discriminatory practice, and health and safety responsibilities.
