We're looking for a dedicated Care Team Leader Nights to join our family at Chartwell House Care Home! Up to 46 hours per week over 7 days – every other weekend. We cannot sponsor any candidates for this role who require sponsorship.

Job Description

As Care Team Leader you will lead a team to assist residents with all aspects of daily living, promoting independence, dignity, and respect at all times. You will be responsible for the smooth running of the shift; you will assign specific tasks to the care assistants and monitor their completion. You will develop, review and update the residents' personal care plans to meet their changing physical, social and psychological needs, complete risk assessments, arrange ongoing care reviews with family and/or representatives and other medical professionals.

Key Responsibilities

  • Lead the care team to provide individualised care and personal care to residents in all aspects of their daily living.
  • Lead and assist the care team in supporting residents at mealtimes, in accordance with their individual needs.
  • Assist residents with those activities which they would usually perform or have in the past performed themselves but cannot now without assistance. The optimum level of independence should be encouraged and supported whilst maintaining dignity and self‐esteem of the residents.
  • Contribute fully to team working, responding positively to colleagues and undertaking all reasonable work instructions promptly.
  • Communicate with external professionals regarding care delivery and attend reviews/meetings/rounds when appropriate or requested.
  • Have a working knowledge of safeguarding, DOLS and MCA and be able to put this knowledge into practice and seek guidance and support as required.
  • Act as a role‐model to Care Assistants, lead and develop key worker system within the home.
  • Ensure that Care Assistants in your team follow the individualised care plans for residents and encourage them to contribute towards their development and review.
  • Be aware of and understand the concept of individualised care. Endeavour at all times to ensure that the care given by your team members is in accordance with the Philosophy of Care.
  • Ensure that confidentiality of residents, their families and friends is maintained at all times in all matters.
