Bournemouth East Collaborative Primary Care Network has a new and exciting opportunity for an ANP to join the BEC Enhanced Care Team. We are looking for an ANP to work 15-30 hours per week providing acute visits as part of our multi-disciplinary visiting team. Wednesday, Thursday and Friday are key required days for the team, Monday and Tuesday would be preferable but not essential.

Main duties of the job:

  • Our Enhanced Care Team is a multi-disciplinary team based at Southbourne Surgery that provides support to our aligned Care Homes, our proactive frailty support and acute visiting service to housebound patients.
  • The role will predominantly involve delivering acute visits to our housebound patients but will also involve offering training and support to our aligned Care Home staff, as well as a broader education and development for the communities served by the PCN.
  • Advanced Practice is a level of practice rather than a type of practice and the ANP within Bournemouth East Collaborative PCN will have the freedom and authority to act, making autonomous decisions in the assessment, diagnosis, and treatment of patients.

Bournemouth East Collaborative Primary Care Network, situated on the stunning Dorset South Coast, comprises four like-minded practices working together in East Bournemouth, with a strong reputation on quality improvement and investing in its employees. The PCN serves a population of approximately 55,000 patients with a diverse demographic.

  • Littledown Surgery

Job responsibilities:

  • Able to use an advanced level of professional accountability, autonomy and judgement for seeing, treating and assessing patients who present with a broad range of situations and conditions.
  • Plan and manage complete episodes of care, working in partnership with other members of the multi-disciplinary PCN team, delegating and referring as appropriate, to optimise health outcomes for patients.
  • Work autonomously, clinically examining and assessing patients and initiating any appropriate action whilst taking responsibility for managing practice.
  • Provide support to Care Home staff which may include delivering training where required, or supporting Care Home pilots, and support the other members of the Enhanced Care Team.

Person Specification:

  • Advanced patient assessment including comprehensive history-taking and physical examination
  • Ability to formulate differential diagnoses and implement advanced clinical decision making
  • Ability to independently prescribe safely and appropriately across a range of conditions
  • Skills in managing clinical risk, recognising red flags, and escalating appropriately
  • Competence with common investigations (e.g., ECG, urinalysis, blood tests, X-ray interpretation commensurate with role)
  • Ability to act as a clinical leader and positive role model
  • Ability to plan and deliver educational sessions to patients, health staff, care home staff and voluntary groups
  • Ability to critically appraise evidence and integrate research into practice
  • Ability to collect, analyse and interpret clinical data
  • Highly developed communication skills, able to manage sensitive and complex conversations
  • Ability to build therapeutic relationships with patients and carers
  • Ability to work collaboratively across organisational boundaries
  • Ability to maintain accurate, legally compliant and high-quality documentation
  • Competent in using clinical systems (SystmOne)

Experience:

  • Has previous experience working at an advanced level in primary care, urgent care or emergency care.
  • Evidence of working at an Advanced level.
  • Experience of managing a range of primary/urgent care conditions for adult and paediatric patients.
  • Experience managing undifferentiated, undiagnosed and complex patients across the lifespan
  • Experience of autonomous caseload management including diagnosing, prescribing and decision making
  • Experience delivering teaching, mentorship or supervision
  • Experience in service development, quality improvement or audit
  • Previously worked as part of an integrated team
  • Experience in home visiting within primary or community care
  • Experience in coordinating or contributing to MDT meetings

Qualifications:

  • Current UK Professional Registration with the NMC
  • MSc advanced clinical practice or equivalent ACP qualification mapped to HEE framework.
  • Independent and supplementary non-medical prescriber
  • Evidence of on-going CPD and reflective practice
  • Advanced Practice Digital badge.
  • Have use of own vehicle with business insurance and clean full UK driving licence.
  • Teaching, mentoring or clinical supervision qualification
  • Leadership or Management Training (E.g. ILM, coaching, QI)
