Starting salary for this role is £34,434 (pro rata for part‑time colleagues). This role sits within a well‑defined pay band, offering clear progression as you grow your career at BCP. Role Purpose

We are based within University Hospitals Dorset (UHD) and work across health and social care services. This post is predominantly located at the Bournemouth Hospital site but also involves supporting teams across all acute and community hospitals.

Main Responsibilities

  • Undertake comprehensive Care Act assessments focusing on the needs of BCP residents over 18 and their carers.
  • Support and enable people to minimise dependence and maximise independence through strength‑based, self‑directed support.
  • Work alongside reablement, intermediate care teams, commissioned providers and monitor and review care packages to meet individual needs.
  • Manage a caseload of visits to residents in hospital settings, care homes and their own homes.
  • Collaborate closely with hospital staff and other provider organisations.

Ideal Candidate

  • Recent experience working within Adult Social Care or a similar agency, with direct contact with members of the public.
  • Understanding of the social and legal context of social care service delivery.
  • Excellent communication skills and the ability to advocate for adults to achieve their outcomes, including signposting, providing information and advice.
