Key Responsibilities:
- Support adults aged 18+ living with long‑term conditions to live safely and independently in the community.
- Work alongside individuals, carers and families to understand what matters most and identify needed support.
- Provide rights‑based practice, court work, safeguarding and other services including links to national and local resources.
- Document and review service plans, risk assessments and statutory reports.
Qualifications & Experience:
- Social work qualification (DipSW, BSc/BASW, MSW, SWDA, or equivalent) and registration.
- Experience working with adults with long‑term conditions or in a similar setting preferred.
Flexibility & Working Arrangements:
- Full‑time, part‑time and flexible (hybrid) options are available. Home working and flexible office spaces.
