About the role:
Community Reablement Assistant working alongside Occupational Therapists and Reablement Officers to help individuals regain confidence and independence after a hospital stay. First point of contact for those returning home.
What you’ll do:
- Supporting people with personal care and empowering them to do as much as possible on their own.
- Monitoring and providing feedback on the progress of people’s goals.
- Assisting with medication and encouraging independence.
- Completing care records and other relevant paperwork.
- Facilitating access to community groups and services.
A valid UK driving licence and use of a car for work purposes are required as the role involves visiting people in the Bournemouth, Christchurch and Poole area.
Weekend Roles Available:
- Post 1: Week one: 09:00‑14:00 & 17:00‑22:00 Saturday & Sunday (20 hours); Week two: 15:00‑22:30 Saturday & Sunday (14 hours).
- Post 2: Saturday and Sunday 07:00‑14:30 (14 hours).
