About the role:

Community Reablement Assistant working alongside Occupational Therapists and Reablement Officers to help individuals regain confidence and independence after a hospital stay. First point of contact for those returning home.

What you’ll do:

  • Supporting people with personal care and empowering them to do as much as possible on their own.
  • Monitoring and providing feedback on the progress of people’s goals.
  • Assisting with medication and encouraging independence.
  • Completing care records and other relevant paperwork.
  • Facilitating access to community groups and services.

A valid UK driving licence and use of a car for work purposes are required as the role involves visiting people in the Bournemouth, Christchurch and Poole area.

Weekend Roles Available:

  • Post 1: Week one: 09:00‑14:00 & 17:00‑22:00 Saturday & Sunday (20 hours); Week two: 15:00‑22:30 Saturday & Sunday (14 hours).
  • Post 2: Saturday and Sunday 07:00‑14:30 (14 hours).

This extends to respite care for families and carers. We operate services across Bournemouth, Christchurch and Poole, ranging from residential homes, day services, supported living sites and reablement home care.
