Are you a motivated and dedicated nurse with Chronic Disease skills/qualifications looking to work in a friendly and professional GP Practice? If so, we would be interested to hear from you. We are seeking a professionally dedicated nurse to lead Chronic Disease in the Practice and to contribute, as necessary, to the wider Practice Nursing Team effort in support of patients. We can be flexible in the job requirements in order to accommodate the right candidate.
Main duties of the job:
- To assist in chronic disease management of our Asthma & COPD patients including, where necessary, referring patients to secondary/specialist care.
- To implement and evaluate individual treatment plans for chronic disease patients that promote health and well-being.
- To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients' health.
- To contribute to the overall effort of the Practice nurse team including, but not limited to, services such as: cytology, ECG, routine Imms & Vaccs, dressings etc.
- To request and process pathology services as necessary.
- To maintain accurate clinical records in conjunction with existing legislation.
- To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
- To support the clinical team with all safeguarding matters.

Boultham Park Medical Practice is an urban GP Practice located just to the south of the centre of the city of Lincoln. We are a training Practice and our core clinical team comprises 2 GP Partners, 1 Pharmacist Partner, 4 salaried GPs, 1 Advanced Clinical Practitioner, and a team of 3 Practice Nurses, 1 Nursing Associate and 3 Healthcare Support Staff. The clinical team is supported by the reception and admin support teams. We are a hard working mutually supportive team, renowned for being professional and friendly. We are looking for the successful candidate to be of the same calibre and with a similar work ethic in order to maintain and enhance team cohesion and effectiveness.
Job responsibilities:
The successful candidate will be professional, motivated and able to work collaboratively as part of our friendly Practice multidisciplinary team. On appointment you will ensure nursing services are delivered effectively to the entitled patient population. The successful candidate will, dependent on skills and experience, take a share of the nursing responsibilities within the Practice which may include areas such as support in infection prevention and control, health promotion, chronic disease management, health prevention, as well as supporting the practice management team in reviewing and delivery of clinical policy and procedure.
