Hours: 39 hours per week

About the role

As a Care Co-Ordinator, you'll play a vital role in ensuring the delivery of high-quality, person-centred care that reflects the individual needs, preferences, and dignity of each resident. Working closely with residents, families, care teams, and external professionals, you will coordinate assessments, care planning, and ongoing monitoring to promote positive health and wellbeing outcomes. You will lead by example, upholding the organisations values: be brilliant, be vibrant, be creative, be welcoming, and be determined while promoting equality, diversity, and a compassionate approach to care.

About you

You will hold a Level 3 qualification in Health & Social Care and be willing to work towards Level 4/5. You must have experience in a care role and a good understanding of the needs of older people, CQC standards, and health and safety responsibilities. You will be able to work both independently and as part of a team, with good communication, literacy, numeracy and IT skills, and the ability to organise and complete tasks to a high standard. Supervisory experience and Moving & Handling Key Trainer training would be desirable.

We are looking for someone who is friendly, respectful and compassionate, with a genuine passion for supporting older people and promoting privacy, dignity and high standards of care. Flexibility to work shifts, including evenings, weekends, public holidays and on call is required.
