The Manager, Care Management provides day-to-day oversight, coordination, and management of functions and resources for the Care Management Department. This role collaborates with Care Management Leadership and colleagues to standardize practices and responsibilities, blending clinical and financial components to achieve optimal patient outcomes and organizational goals.
Essential Functions- Provide leadership and supervise day-to-day Care Management operations.
- Promote standardized practices related to Advance Care Planning, length of stay (LOS) management, readmission prevention, use of predictive readmission tools, proactive care progression, addressing barriers (including tracking avoidable delays), daily team rounding, care transitions, and patient satisfaction related to transitions.
- Monitor success using qualitative and quantitative data and take action to improve outcomes.
- Advocate for right care at the right time and place.
- Educate care management staff, physicians, nurses, and other team members on progression of care, level of care (LOC), and safe, timely transition management.
- Support national standards for care management scope: Education, Care Coordination, Compliance, Transition Management, and Resource Utilization.
- Utilize Care Management systems to support proactive care across the continuum.
- Align with population health and community health efforts to support a patient-centered model.
- Collaborate with ambulatory and post-acute providers to ensure seamless transitions of care.
- Partner with physician leadership to optimize utilization data through committees and analytics platforms (e.g., Quality Advisor and similar resources).
- Direct daily operations to optimize personnel utilization aligned with patient and ministry needs.
- Assist in developing annual departmental goals, strategic plans, and initiatives.
- Foster collaboration with nursing, quality, hospitalists, and physician leadership to support clinical integration and high-quality, cost-efficient care.
- Participate in developing and managing the annual budget.
- Identify and achieve cost reductions.
- Work with colleagues to standardize operations and understand the financial impact of contracts and payment models from fee-for-service to value-based care.
- Collaborate with stakeholders to reduce cost.

Education- Bachelor of Science in Nursing or Social Work (required)
- Masterβs in Nursing, Social Work, or Healthcare Administration (preferred for BSMH; required for RSFH)
- 3 years of recent acute hospital care management experience (required)
