Position Summary We are seeking a diligent and organized Transition Coordinator to join the Integrated Project Team (IPT) responsible for overseeing medical and medical research equipment during construction, outfitting, and transition phases at Portsmouth and within the Defense Health Agency (DHA) Facilities Enterprise. This role is focused on supporting Major Repair and Renovation (MRR), SRM, and MILCON projects by managing transition efforts, coordinating equipment installation, and ensuring seamless project completions.
Essential Duties And Responsibilities:
- Actively participate as part of the IPT during construction, initial outfitting, and transition phases relevant to medical and research equipment.
- Provide on‑site support for transition and equipment projects.
- Develop, maintain, and update tracking systems and spreadsheets for outfitting and transition requirements.
- Identify transition and initial outfitting needs early in project planning; assist facility staff, end‑users, and contractors to coordinate procurement, installation schedules, and ensure timelines are met.
- Attend project and transition meetings, take minutes, and maintain written records of all discussions.
- Collaborate with the Transition Committee chairman to develop meeting agendas, document follow‑up actions, and track progress.
- Prepare and submit required project approval documents and reports in compliance with DHA regulations.
- Be available via pager or cellular phone during project activities and respond promptly to on‑site issues.
- Maintain thorough project files and documentation to ensure proper project close‑out.
- Monitor and coordinate Major SRM and MILCON projects at Portsmouth; may provide remote support to other DHA sites following coordination with the Contracting Officer Representative (COR).
- Provide monthly task reports and detailed meeting records including attendee lists.
Education, Experience, And Technical Skills Required:
- Bachelor’s degree, or related field with minimum 5 years experience.
- Certifications are a plus or relevant industry credentials (e.g., PMP, CCM).
- Experience in healthcare, government, or organizational transition planning.
- Must have or be able to obtain and maintain a Real ID or Passport in order to undergo commercial airfare travel.
- Must have or be able to obtain and maintain a valid driver’s license.
Working Environment This position operates in a professional office setting. The role routinely uses standard office equipment such as computers, phones, printers, and photocopiers. The work environment is generally quiet and free from exposure to loud noise, hazardous materials, or heavy machinery. Employees are typically seated for extended periods and may occasionally move about the office to attend meetings or access equipment and supplies.

- Sitting/Standing: This role is generally sedentary, performing sitting work utilizing technological office equipment including a laptop, mouse, keyboard, computer monitors, telephones, scanners, and copiers.

- Stooping/Kneeling/Crouching/Crawling: This role may require stooping to reload printer trays with print paper, moving equipment, and managing cords.
- Using Hands/Fingers: This role constantly operates technological office equipment such as laptops, mice, keyboards, computer monitors, telephones, scanners, and copiers.
- Talking/Hearing: This role requires constant communication with others in the organization to exchange information, review information, and make decisions about information.

- Carrying/Lifting: This role requires lifting up to 20 lbs to transport technological office equipment to and from various workspaces.
