You will be responsible for delivering tailored support to individuals attending Pulmonary Rehabilitation sessions in community clinics, working within clinical protocols and guidance. As well as assisting in the setup, delivery and evaluation of rehabilitation programmes, you will help ensure sessions are welcoming, accessible and person‑centred. You will monitor patient progress, adapt support as needed and communicate any concerns or developments to the clinical team. You will manage a caseload with care and efficiency, prioritising needs to provide timely and inclusive support. As part of discharge planning, you will help patients build confidence and develop long‑term self‑management strategies. You will contribute to a collaborative and respectful environment, supporting coordinated care across the multidisciplinary team.

What Makes You Great

  • Strong foundation in literacy and numeracy demonstrated through formal education.
  • NVQ Level 3 in Health and Social Care, Diagnostic and Therapeutic Support or an equivalent relevant qualification.
  • Evidence of recent professional or personal development in a clinical or healthcare setting.
  • Full UK driving licence to support community‑based patient care.
  • Level 4 Certificate in Exercise Training, ideally focused on Chronic Respiratory Disease.
  • Degree or Diploma in Exercise Physiology with clinical application.
  • NVQ Level 4 or Foundation Degree in a clinically relevant subject.
