Summary
We are seeking a Community Care Lead to join our team supporting customers across the West Dorset and Purbeck area.
This vital role involves liasing with customer and their families, ensuring high standards of care, and supporting our dedicated staff.
Your work will directly contribute to enhancing the lives of those we serve by providing compassionate and tailored care.
Responsibilities
- Lead and coordinate community care team member to deliver exceptional home care services.
- Develop and oversee personalised care plans for customers.
- Supervise and support care staff, fostering a positive team environment.
- Ensure compliance with all relevant legislation and company policies.
- Promote a culture of continuous improvement and innovation.
- Build strong relationships with clients, families, and healthcare professionals.
- Monitor service quality and implement improvements as needed.
- Manage administrative tasks related to care delivery using IT systems.
- Take part in the Out of Hours phone rota
- Proven leadership and supervisory experience within the care sector.
- Minimum of H&SC Level 2 and be prepared to work towards Level 3
- Strong communication skills with the ability to motivate and support staff.
- Experience in developing and managing care plans.
- Valid driving license and own car is essential.
- Knowledge of relevant legislation and best practices in home care.
- Proficiency with IT systems used in healthcare settings.
- Ability to work independently and as part of a team.
- Passion for delivering compassionate, person-centered care.
