Are you looking for a new challenge that will develop your leadership and team‑working skills? We are seeking a highly skilled, enthusiastic leader to lead our Frailty Team at Clarendon and the Community Falls Team at Washington.
Responsibilities
- Accountable for the overall management, development and line management of the Frailty and Community Falls Teams.
- Lead, motivate and support a team approach to delivering innovative models of care.
- Ensure systems are in place to support clinical governance, quality monitoring and ongoing research and audit of practice.
- Maintain patient safety and facilitate the development of clinical expertise within the team.
- Work collaboratively with multidisciplinary teams to ensure evidence‑based and safe practice.
Qualifications and Experience
- Registered Nurse or Advanced Health Professional with degree‑level training and post‑registration qualification specific to the role.
- Experience within a community setting as a Nurse Practitioner, Community Matron or similar caseload holder.
- Significant experience at Band 6 level or above in a community setting, with portfolio of continuous professional development.
- Advanced clinical skills, non‑medical nurse prescribing and ENCOP (Foundation or specialist level).
- Leadership experience and ability to undertake audit, service review and development.

- Leadership and interpersonal skills.
- Excellent communication skills, able to forge relationships with colleagues and stakeholders.
- Analytical and decision‑making skills, with expertise in assessment and clinical decision making.
- Ability to work under pressure and achieve tight deadlines in a complex, changing environment.
