We are looking for a passionate Social Worker to join Blaenau Gwent's Community Mental Health Team at the Cwm Coch, Ysbyty Aneurin Bevan site. This role offers an opportunity to work within a multi‑disciplinary team in a fast‑paced environment, helping vulnerable adults with serious mental illness improve their outcomes.

Key Responsibilities

  • Conduct comprehensive assessments and craft individual care and treatment plans in accordance with the Mental Health (Wales) Measure (2010).
  • Collaborate across the multi-disciplinary team at Cwm Coch, sharing expertise and supporting holistic, person-centred care.
  • Serve as a key advocate for recovery and independence for clients, building and maintaining strong relationships with individuals, families and support networks.
  • Implement and oversee strategies to promote safety, wellbeing and ongoing progress, providing guidance during challenging situations.

Qualifications & Requirements

  • Registered Social Worker with an active professional registration.
  • Desire or experience as an Approved Mental Health Professional (AMHP) – or willingness to pursue AMHP training.
  • Experience or commitment to working with adults living with serious mental illness.
  • Ability to communicate effectively, collaborate within a team and approach problem‑solving proactively.
  • Access to a private vehicle for work purposes.

This post is exempt from the Rehabilitation of Offenders Act 1974. A comprehensive pre‑employment screening process will be undertaken, including an Enhanced Disclosure with Child and Adult Barred List.
