This is an exciting opportunity to join Blaenau Gwent's Home Care Team as a Functional Assessor — a role that's all about making a meaningful difference every day. You'll work with individuals to understand their needs, support their recovery, and help them regain confidence in daily living, all while being part of a friendly, forward‐thinking team that truly values your contribution.
As a Functional Assessor, you'll play a key role in helping individuals maintain or regain their independence within their own homes. Working as part of the Home Care STARS Team, you'll carry out short‐term assessments of need to identify the right care and support that enables citizens to live well, safely, and with dignity. Your work will involve close collaboration with professionals across health and social care — from therapists and nurses to community resource partners — to ensure every individual receives joined‐up, person‐centred support. You'll monitor progress, adjust care plans, and respond proactively to changes in a citizen's wellbeing, ensuring that care remains effective, safe, and empowering.
We're looking for someone who's caring, confident, and committed to helping individuals live independently. You'll bring compassion, patience, and excellent communication skills to every interaction, helping individuals and their families feel supported and reassured throughout their care journey. You'll need to be well organised, able to manage your own workload, and comfortable making decisions based on assessment and observation. Experience in care or health‐related roles is beneficial, but just as important is your empathy, positivity, and willingness to learn. You'll work as part of a supportive multidisciplinary team, so being a team player is key — someone who values collaboration and takes pride in delivering excellent care. Most of all, you'll have a genuine passion for making a difference in people's lives and empowering them to reach their potential.
