Your role will be to help vulnerable people maintain and regain their independence and remain in their own home with an improved quality of life. The role also involves supporting individuals upon discharge from hospital as part of the Discharge to Assess pathways and Complex and Palliativecare.
The successful candidates will be working on a rota, including weekends, bank holidays and shift working. This is an excellent opportunity for the right people to work in a modern, caring, fulfilling service, in return we offer excellent staff support systems, training and development. Although previous experience could be an advantage, it is not essential. We are able to be flexible in the contracted hours offered and preferences can be discussed at interview.
