Work with our growing Advanced Clinical Practitioner team within the Rapid Response Team to provide assessment and interventions to prevent hospital admissions, facilitate discharges meeting the 2‑hour Urgent Community Response standard, and support patients on the Frailty and General Virtual Wards.

Main duties of the job

We're recruiting an ACP to deliver NHS England's Community Agenda. You will support people in their usual place of residence during a crisis, treating them at home when safe to reduce unnecessary hospital admissions. As an Advanced Practitioner you will bring experience of working collaboratively within a community or acute setting, exceptional assessment, intervention and communication skills. You will conduct comprehensive assessments, make autonomous decisions, and serve as a key decision‑maker on the patient’s pathway during and after crisis. You will offer remote advanced consultation, clinical advice and supervision to team staff. We provide interdisciplinary induction, clinical supervision, and links to Primary Care Networks for prescribing and clinical support.

Clinical Responsibilities

  • Assess patients with undifferentiated and undiagnosed problems and determine their health‑care needs.
  • Screen for disease factors and early signs of illness.
  • Make differential diagnoses using strong decision‑making and problem‑solving skills.
  • Develop care plans for health and well‑being to be implemented during and after crisis.
  • Order necessary investigations and provide treatment and care individually or as part of a team, including referral to other agencies.
  • Admit or discharge patients from the caseload and refer to other health‑care providers when appropriate.
  • Collaborate with other health‑care professionals and disciplines, providing leadership and consultancy as required.
  • Support patients on the Virtual Ward, working with the MDT.

Qualifications

  • Clinical based professional degree.
  • Current registration with the NMC or HCPC.
  • Advanced Practice MSc or NHS England credentialing via the supported e‑portfolio route.
  • Management/leadership qualification or attendance at equivalent study days (master’s qualification or equivalent).
  • Non‑medical prescriber (V300).
  • Evidence of continuous professional development in line with the four pillars of advanced practice.

Experience

  • Extensive professional/clinical knowledge managing unwell patients in the community, primary or acute setting.
  • Experience of patient history taking, clinical examination and developing management plans.
  • Managing own caseload.
  • Knowledge and experience of managing chronic disease and long‑term conditions.
  • Effective communication of complex and highly sensitive information.
  • Experience of leading service development and quality improvement work.
  • Effective people management and leadership skills.
  • Experience of teaching staff.
  • Involvement in policy and practice change.
  • Experience in a senior position within an appropriate health‑care field.
  • Mentorship and development of nurse and AHP practice.
  • Experience of managing staff.
