Blackpool Council is recruiting a Transfer of Care Co-ordinator to be the pivotal link between hospital and home within our Home Care and Reablement Service. You will coordinate referrals, deploy care teams and ensure timely, high-quality support for people transitioning from hospital.
Working with health professionals, social care teams and families, you will lead daily service huddles, monitor capacity and support performance reporting while providing coaching to colleagues.
