Role Overview
We are pleased to offer a full time Social Work vacancy within the Community Mental Health Team for Older Adults (OACMHT). The successful candidate will join a well-established and supportive team of Social Workers, Case Assessor and Support Workers. We are a co-located team working with older adults with various mental health conditions and their family/representative. We are committed to a joint approach to service planning, delivery and development through working alongside our health colleagues in an integrated way that is well established within the team and across wider services.
Responsibilities
For Older Adults, you will provide multi-disciplinary needs and risk assessments, care planning and long‑term social work support within the framework of the Care Act 2014. Your practice will be underpinned by other legislative frameworks, local policy and will be in line with the national agenda driving the provision of care for older adults with a mental health condition. Additionally, you will be committed to your professional development and role as a Social Worker in line with Social Work England protocols.
Qualifications & Experience
We welcome applications from both experienced and newly qualified Social Workers. Approved Mental Health Professional status would be advantageous, but not essential.
